Building a Healthier Community

About Us

The West Elgin Community Health Centre delivers primary health care, illness prevention and health promotion services, as well as a range community programs to the residents of western Elgin. We believe that effective primary health care addresses the determinants of health, which include access to shelter, education, food and stable eco-systems. We use a community development approach to building healthy public policy in supportive environments.

Our vision is rooted in a care model that provides comprehensive primary care services, delivered by interdisciplinary teams of professionals practicing within a health promotion framework. This means that by working with individuals, families and groups, we increase individual and group capacity in building healthy communities. We deliver a range of integrated community-based services and respond to health-related community concerns. We are specialists in delivering primary health care that is integrated with other social and health services partners.

Our care model is highly effective. It is a resource for people who encounter a diverse range of access barriers such as language, literacy, poverty and geography. It also works for those with other social-cultural barriers and who are at high risk for developing health problems.

We believe the most effective way to provide primary health care is through integrated teams of community-focused professionals from all health disciplines. Our Primary Health, Community Health, Early Years, Teens and Youth, Seniors and Mental Health services are all delivered by interdisciplinary teams that strive to provide care continuity and coordinated  services to all individuals and families served by the Centre.
