Building a Healthier Community


Want to become a registered client?


Registered clients are individuals who currently use the services of the Primary Health Care Team. The process of becoming registered means you:

  • Will complete an application form to become registered
  • Will have your name go on a wait list (names are prioritized based on need)
  • Will be assigned to a physician or nurse practitioner based on your health history and current needs
  • Will be called for an intake interview with a physician or nurse practitioner where:
    • You can tell us more about what you are seeking in a provider
    • We will share with you what we can offer here at West Elgin Community Health Centre, how we work and if we can meet your needs


Once it is agreed that you will become a registered client, you will be booked in for a physical examination and will be asked to sign a consent to have your past medical records sent to us if possible.

As a Registered Client you:

  • Are assigned to one of our providers (3 physicians or 3 nurse practitioners)
  • Can be seen by another provider if the one you are assigned to is unable to see you
  • Are able to utilize the on-site lab services we offer here in conjunction with Gamma- Dynacare
  • Are able to participate in the Free Flu Vaccine Clinics held at the Centre each fall
  • Are able to access the after hours on-call service provided by our physicians and nurse practitioners
  • Are able to access all Programs and Services offered here at WECHC


Enrolment:
There are changes occurring within the health care system in Ontario. Why is this change necessary? A number of people, who access the health care system, are seeing more than one doctor (visiting walk-in clinics and emergency rooms for non-urgent illnesses) and this is increasing the cost of providing health care to people in our province and city.

The provincial government is introducing several new changes and we are doing our part to help manage health care so that it will continue to be available when people need it. One of the changes that is being implemented is Enrolment. This means that all our clients who access services from a doctor or nurse practitioner at West Elgin Community Health Centre are invited to consider enrolment with West Elgin Community Health Centre. Enrolling with us means that you agree to use our services as your main care provider.

The benefits of enrolling with West Elgin CHC:

  • Better access to the information needed to provide quality healthcare
  • Access to a Telephone Health Advisory Service (THAS) which can be provided in over 100 languages once enrollment process is complete
  • With your permission, your doctor or nurse practitioner will be kept up to date about your health care needs even if you see a doctor in an emergency room
  • The health care system will increase its focus on helping you stay healthier longer
