The West Elgin Community Health Centre and St. Joseph’s Health Care, London have partnered to provide the Primary Care Asthma Program. The Program offers assessment, treatment, education and follow-up care for adults and children who have asthma.
Care and treatment is based on best practices, following the Canadian Asthma Consensus Guidelines, and is facilitated by a Certified Asthma Educator. This is a new and growing program, so speak with your doctor or nurse if you are interested. Evening appointments are available.

Who can Access this Program/Service
- Individuals with asthma who live within our catchment area
- A referral from a doctor or nurse practitioner faxed or phoned to us is necessary

The 30 Second Asthma Test:
- Do you cough, wheeze, or have a tight chest because of your asthma? (4 or more days a week)
- Do coughing, wheezing or chest tightness wake you at night? (1 or more times a week)
- Do you stop exercising because of your asthma? (in the past 3 months)
- Do you ever miss work or school because of your asthma? (in the past 3 months)
- Do you use your blue inhaler 4 or more times a week? (except one dose/day for exercise)
If you answered “yes” to one or more questions, your asthma may not be controlled as well as it can be. Talk to your doctor or nurse. Your asthma can be controlled!
