Who We Are?
West Elgin Community Health Centre is a non-profit community based organization. We have diabetes teams located in:
- West Lorne at the West Elgin Community Health Centre
- Straffordville at the Bayham Medical Centre
- Aylmer above the Shoppers Drugmart
- Dutton at the Dutton Medical Centre
The education teams consist of a Registered Nurse (RN) and Registered Dietitian (RD) specializing in diabetes care and teaching individuals how to self manage their diabetes.
What We Do?
The Diabetes Education Centres:
- Provides free individual counselling and group education sessions for adults (18 years and older)
- Works with people who have Type 1 and Type 2 diabetes or pre-diabetes (people with blood sugars between 6 and 6.9)
- Works with family members and caregivers
- Have a registered nurse and a registered dietitian who facilitate the classes and provide individual counselling
- Assist you to manage your diabetes

Who Needs Our Services?
Adults 18 years and older who:
- Have Type 1 , Type 2 diabetes or pre-diabetes
- Want to add some variety to their meals and still be able to keep their sugars well controlled
- Want to learn more about how what they eat will effect their sugars
- Want to understand more about what makes their sugar levels rise/fall
- Want to learn more about how exercise - including walking - can help to manage sugar levels
- Want to meet others living with diabetes
Who can Access this Program/Service?
- Adults who have been diagnosed with diabetes (Type 1 or 2) or pre-diabetes
- Adults who live in Elgin County or are aware of services in their area but would prefer to access Elgin County Services for other reasons (close to where you work etc.)
- You do not need to be a registered client of the West Elgin Community Health Centre
- We will work closely with your physician or nurse practitioner but you do not need a referral from them to see us
